This teacher observation form is designed for K-12 instructional leadership experts to evaluate and provide feedback on various aspects of a teacher's performance. The form is divided into sections including Instruction, Planning, Environment, and Professionalism. Within each section, there are specific questions related to standards and objectives, motivating students, presenting instructional content, lesson structure and pacing, activities and materials, questioning, academic feedback, grouping students, teacher content and knowledge, teacher knowledge of students, thinking, problem-solving, instructional plans, student work, assessment, expectations, managing student behavior, and creating a respectful culture. The form allows for both objective and subjective feedback, with space for comments on each question as well as an overall qualifying measure and comments section. The goal of this form is to provide constructive feedback to teachers to improve their instructional practices and create a positive learning environment for students.
