Mosaic
This teacher observation form is designed to evaluate a teacher's instructional activities and their impact on students. It includes sections for oral and/or written feedback, teacher activity during instructional activities, student feedback, and final comments. The questions within each section are focused on academic performance, frequency, and quality of feedback, timing of feedback, teacher activity during instruction, student-to-student interaction, and reinforcement and refinement of student work. The form is intended to be regularly used to monitor and adjust instruction, encourage student engagement, and facilitate student-to-student interaction. The form also includes a SCTS 4.0 rating to provide an evaluation of the teacher's performance.
