This teacher observation form is designed to assess various aspects of a classroom visit. It includes questions related to the standard/objective for the lesson, the clarity of the learning objective, the approximate entry time of the lesson, class size and configuration, on-task students, rigor and relevance of the lesson, checking for understanding, teacher feedback, established transitions/routines/procedures, classroom management, instructional format, student activity, student/teacher interaction, and assessment.
