This teacher observation form is designed to assess a teacher's ability to plan and deliver effective instruction. It includes sections on long-range planning, short-range planning of instruction, planning assessments and using data, establishing and maintaining high expectations for learners, using instructional strategies to facilitate learning, providing content for learners, monitoring, assessing, and enhancing learning, maintaining an environment that promotes learning, managing the classroom, fulfilling professional responsibilities, and additional notes/comments. The questions within each section are focused on specific aspects of planning and instruction, such as analyzing student information, setting appropriate goals, developing instructional plans, and using data to guide instruction. The form is intended to provide a comprehensive evaluation of a teacher's instructional practices and help identify areas for improvement.
