This teacher observation form is designed to assess instructional practices in a K-12 classroom setting. It includes questions about basic information such as the number of students, grade level, subject/content, materials/resources, instructional setup, and technology. The form also asks whether the learning target is posted and effectively used in instruction, as well as the extent to which strategic teaching strategies are employed. Effective questioning, the presence of current student work/word walls/academic vocabulary, and the students' ability to verbalize the learning target/I can statement are also assessed. The form concludes with an open-ended section for additional comments.
