The teacher observation form is a tool used by instructional leaders in K-12 schools to assess and evaluate teachers' performance in the classroom. It consists of several sections and questions that focus on different aspects of teaching and learning. The form includes sections to assess the students' engagement and participation in various math activities, such as working in small groups with the teacher or other students, using math centers or stations, or participating in whole group lessons. It also includes a section to evaluate the instructional materials used by the teacher, such as math manipulatives, practice cards, math games, iPads or computers, and traditional tools like pencil and paper. The form further incorporates the 8 Mathematical Practices, which are essential skills and habits of mind in mathematics education. These practices include making sense of problems and persisting in solving them, reasoning abstractly and quantitatively, constructing viable arguments and critiquing the reasoning of others, modeling with mathematics, using appropriate tools strategically, attending to precision, looking for and making use of structure, and looking for and expressing regularity in repeated reasoning. Lastly, the form includes a question about whether small group planning is evident in the lesson plans. This question aims to assess the teacher's ability to effectively plan and differentiate instruction for small groups of students. It is important to note that this teacher observation form may not cover all aspects of teaching and learning, but it provides a structured framework for instructional leaders to evaluate teachers' performance in the classroom.
