This teacher observation form is designed to assess the effectiveness of Tier II interventions in K-12 schools. It includes questions related to the grade level, program, skills, and teacher qualifications. The form also evaluates whether the intervention targets a specific area of need and occurs in addition to Tier I instruction. It assesses whether the intervention is delivered in a small-group format, with fidelity, and with evidence-based materials. The form also evaluates whether the intervention is provided with the appropriate amount of time daily and weekly and is progress monitored at least every other week.
