This is a form that instructional leaders can use to observe and evaluate teachers during a lesson. It includes various sections and questions that cover different aspects of the lesson, such as the purpose, organization, and content. The form also assesses the teacher's ability to effectively communicate and model procedures, thinking, and expectations. Additionally, it provides feedback on the lesson sequence, including the inclusion or exclusion of essential and non-essential information. The form concludes with a reinforcement/refinement section and a rating system.
