Mosaic
The teacher observation form is a tool used by K-12 instructional leaders to evaluate and provide feedback on a teacher's performance during a specific lesson. The form is divided into several sections, each with a set of questions designed to assess different aspects of the lesson, such as the subject area, grade level, type of observation, lesson plan availability, instructional plans, learning objectives, activator/lesson importance, classroom environment, concept development/skill development, guided practice, transition, independent practice, closure, reinforcement comments, and refinement comments. The form also includes a section for the instructional leader to rate the school's focus on specific areas, as well as a space for photo documentation. The purpose of the form is to provide constructive feedback to the teacher and identify areas for improvement.
