The teacher observation form is a tool used by instructional leaders in K-12 schools to assess and evaluate the effectiveness of co-teaching arrangements. It consists of various sections and questions that focus on specific indicators of effective co-teaching practices. These indicators include the extent to which co-teachers have planned together, the utilization of research-based instructional strategies in the classroom, and the differentiation of lessons in terms of content, process, product, and learning environment. The form also assesses the level of parity between co-teachers, their active involvement during instruction and activities, and their equal sharing of classroom and instructional responsibilities. Additionally, the form examines the presence of routines and formal procedures used by both teachers and students, as well as the level of collaborative and effective teacher communication and interaction observed. The form also includes sections to document the observation of co-teaching instructional arrangements, student instructional grouping patterns, and any additional notes and comments.
