This teacher observation form is designed for K-12 instructional leaders to assess various aspects of a teacher's performance during a lesson. The form is divided into several sections, each focusing on a different area of teaching, such as higher order questioning, meaningful content, differentiated instruction, environment, instruction, student engagement, type of assessment, technology, arts inclusion, lesson plans, and summary. The form includes questions that can be answered at the beginning, middle, or end of a lesson and asks for information about the class gender and grade level. The questions cover a range of topics, including whether the teacher uses higher order questioning, if the content is meaningful and aligned to standards, if differentiated instruction is evident, and if the classroom environment is purposefully designed. The form also includes questions about classroom management, such as whether there is a strong classroom community and positive feedback provided to students, and if current and meaningful student work is posted inside and outside the classroom. Additionally, the form asks about the type of instruction observed, whether students have the resources and time to be successful, and if the teacher uses questions that require a rationale and evidence from student responses. The form also includes questions about personal accountability, auditory/silent cues, classroom procedures, agenda posted, AVID strategies, and whether students are engaged in experimental, hands-on learning. There is space for notes and additional information, and the form concludes with a summary section.
