The teacher observation form is a tool used by instructional leaders to assess and evaluate a teacher's performance in various areas of teaching. It includes questions related to engaging and supporting all students in learning, creating and maintaining effective environments for student learning, understanding and organizing subject matter for student learning, planning instruction and designing learning experiences for all students, assessing students for learning, and developing as a professional educator. The form also includes questions related to specific strategies and techniques that teachers can use to optimize student choice and autonomy, relevance, value, and authenticity, minimize threats and distractions, heighten salience of goals and objectives, vary demands and resources to optimize challenge, and foster collaboration and community. Additionally, the form includes sections for recording the grade and subject being taught, entry time, photos, percentage of students on task, and notes. Overall, the teacher observation form is a valuable tool for instructional leaders to provide feedback and support to teachers in order to improve student learning outcomes.
