This teacher observation form is designed to provide instructional leaders with a structured approach to evaluating teacher performance in the classroom. The form is divided into two main sections: Demographics and Questioning. The Demographics section collects basic information about the class being observed, including grade level, subject, class size, segment, and length of observation. The Questioning section is designed to assess the teacher's ability to engage students in critical thinking and problem-solving through a variety of questioning techniques. The section includes questions related to the teacher's ability to recall, understand, apply, analyze, evaluate, and create knowledge, as well as their use of cold call, student volunteer, student blurt out, choral response, whole class signaling, and written response techniques. The form also includes questions related to the depth of knowledge (DOK) levels being addressed, student input, relational questioning, rigor, and general wait time.
