The teacher observation form is designed to assess the fidelity of guided reading sessions in K-12 classrooms. It includes questions related to the teacher's ability to group students according to data, the presence of lesson plans, informal monitoring of student progress, and engagement of students not at the guided reading table. The form also evaluates the teacher's ability to promote growth by providing appropriately challenging texts to students. The purpose of the form is to provide instructional leaders with a tool to evaluate the effectiveness of guided reading instruction and identify areas for improvement.
