This teacher observation form is designed to assess a teacher's effectiveness in various areas of instruction. It includes sections for gathering basic information about the teacher and the subject area being taught, as well as sections focused on collaboration, relevance, personalization, and intentionality in teaching. The form includes a series of questions designed to evaluate how the teacher interacts with students, the degree to which students are working collaboratively, how students are connected to learning outside of the classroom, whether students are researching and discovering answers to problems, whether students are given choices in how to show their learning, and whether the work is matched to the NCSCOS. The form also includes a section for summary comments, as well as areas for noting positive aspects of the teacher's performance and areas for improvement. Overall, this form is intended to provide a comprehensive assessment of a teacher's instructional practices and effectiveness.
