The form includes a variety of questions that assess the teacher's location, implementation of specific rules, and whether instruction is happening throughout the entire class period. It also evaluates the type of instruction being delivered, the level of engagement of the students, and the type of tasks they are completing. The form also assesses whether the objective is clearly stated on the board and if there is evidence of personalization, relevancy, critical thinking, or innovation in the students' work.
