This teacher observation form is designed to assess the effectiveness of a teacher's ability to motivate students in a K-12 classroom setting. The form includes questions related to the teacher's demographics, the grade level and subject being taught, the size of the class, and the length of the observation. The form also includes questions related to the engagement levels of the students, the types of activities being conducted, and the strategies being used to promote student engagement. Additionally, the form assesses the level of rigor in the classroom and the level of agency given to the students. Finally, the form includes a section for a photo to be taken as evidence of the observation.
