This teacher observation form is designed to assess various aspects of a teacher's instruction and classroom environment. The form includes questions related to the visibility and referencing of objectives, formative and summative assessments of student learning, organization and scaffolding of content, evidence of routines, rigor of student work, and the use of questioning at different levels of cognitive complexity. Additionally, the form evaluates the teacher's modeling of instruction, provision of positive reinforcement, incorporation of students' interests and cultural heritage into lessons, and utilization of the workshop model. The form is intended to provide a comprehensive evaluation of a teacher's effectiveness in promoting student learning and engagement.
