COF Short Form
This form is intended for multiple short observations of classrooms.
Mosaic
The teacher observation form is a tool used by K-12 instructional leaders to assess and evaluate the performance of teachers in the classroom. The form begins with sections for recording the current time and activity in the classroom. It then asks specific questions about what the teacher is doing and what the students are doing during that time. The form also includes a section to assess the level of student engagement and the total number of students present. This process is repeated multiple times throughout the observation period. Finally, there is a section for the observer to provide any final comments or observations.
