This teacher observation form is designed for K-12 instructional leaders to use during a guided reading lesson for grades K-2. It includes sections for the grade level, start of observation, and an assessment of the teacher's preparation before the lesson. The form also includes sections for evaluating the teacher's use of familiar reading/running records, word study, and interactive writing/reading response journal activities during the lesson. Finally, the form includes a section to assess the overall effectiveness of the lesson. The purpose of this observation form is to provide instructional leaders with a clear and objective way to evaluate a teacher's performance during a guided reading lesson.
