This teacher observation form is designed for K-12 instructional leaders to assess various aspects of a classroom environment. The form includes questions related to the grade and subject observed, as well as the period observed. The criteria assessed include the student-centeredness of the classroom, the posting and effective utilization of behavioral expectations and consequences, the room arrangement for whole and small group instruction and interaction among students, the engagement of students in meaningful activities, the use of positive language, the honoring of student voice, and the display of authentic student work. Additionally, the form assesses the organization and cleanliness of the classroom, the appropriateness and attractiveness of bulletin boards, and the communication/posting of objectives/standards. The form also includes a section on the availability of technology for teaching purposes.
