Mosaic
This teacher observation form is designed to assess various aspects of a teacher's instructional practices in K-12 settings. The form includes questions related to the learning connections made in the lesson, whether students understand the purpose of the lesson, the instructional strategies used, the visibility of lesson plans, the level of questioning and student engagement, and feedback on the instructional setting. Additionally, the form includes questions related to collaboration among teachers, connectivity within the classroom, relevance of the lesson to students, and personalization of instruction. The form also includes space for notes on collaboration, connectivity, relevance, and personalization, as well as feedback on the school's CCRP (College and Career Readiness Program).
