This teacher observation form is designed to guide instructional leaders in observing and evaluating teachers in K-12 schools. The form includes several sections with questions related to essential questions and big ideas, the setting of the lesson, depth of knowledge (DOK) levels, whole class and individual instruction, and any other observations or notes about the lesson.
