This teacher observation form is designed to gather information about various aspects of a teacher's classroom instruction. It includes questions about the grade level, subject, and class size of the observed class, as well as the segment and length of the observation. The form also asks about the teacher's use of direct instruction and student talk, both individually and in peer-to-peer interactions. Additionally, it covers the teacher's ability to manage behavior and materials, as well as any administrative tasks performed during the instructional period. The form also includes questions about non-instructional activities, transitions, and provides an option for taking a photo.
