This teacher observation form is designed for K-12 instructional leaders to assess and evaluate teachers' performance in various areas. The form includes sections for ELA skills/strategies/applications, math skills and applications, and checking for understanding. Within each section, there are specific questions related to skills and strategies, application, and observed changes in teaching. Additionally, the form includes various tools and techniques such as accountable talk, buddy journal, character map, diagram the context, exit tickets, give one get one, hand gestures, questioning, RAFT, self-directed response, think/pair/share, and thumb signals for understanding. The form is intended to provide a comprehensive evaluation of a teacher's instructional practices and effectiveness.
