It includes questions about the grade level and subject being taught, as well as the class size, segment, and length of the observation. The form also asks about the types of problem-solving strategies being used in the classroom, as well as the level of rigor being applied to the lesson. Finally, there is a section for taking a photo of the classroom environment.
